INSURANCE INFORMATION

OUT-OF-POCKET PAYMENT POLICY

I will not file a claim with your insurance company. I will provide you with the forms needed to submit a reimbursement claim to your insurance provider for the cost of any consult. Reimbursement is not guaranteed. You may also submit the forms to your health care spending account. Please bring your OB/MW's phone and fax number, along with the contact info for your baby's care provider. I do not require a doctor's order from these care providers, but you will want one to file with your insurance.
